The rates given are for infant deaths per 1,000 live births, reported according to the place of residence, not the place of death.  The data here for the 50 states comes from the National Center for Health Statistics,"Infant Mortality Rates by State.

Infant mortality data given for the top 50 most populous cities comes from Kids Count Data Center, which also uses the most recent Census counts.  The list of cities for which the data is given may change over time. Therefore no data will show up for those cities not reported.
